EXAMPLE: Questionnaire/Database Integration
For this project, information had to be gathered by phone, mail, and online questionnaire, then stored in a centralized database. All aspects of the online questionnaire were designed and created by r.russon consulting.

 
 

The survey begins by asking basic demographic information.
     
  The 300+ item survey is divided into three sections that correspond to tables in the client's database.


The questionnaire is strategically arranged to get the most important information first and to place more personal questions deeper inside so users are less likely to quit.
     
  The questionnaire features automatically calculated totals and several other features to make data entry faster and easier.

On the client-side and server-side, error checking is performed to help insure that data is complete and correct.
